What is orforglipron?
Orforglipron is a small non-peptide molecule that stimulates the receptor of the gut hormone GLP-1. According to the EMA, because it is not a peptide, it was developed to be taken orally; it reduces appetite, increases satiety, and lowers blood sugar by supporting pancreatic insulin secretion and reducing glucagon when blood sugar is high. [10] Semaglutide, liraglutide, and tirzepatide, by contrast, are peptide drugs. For the general characteristics of the class, see GLP-1 agonists.
According to the FDA label, the tablet can be taken once daily with or without food; treatment starts at a low dose and is increased gradually at intervals of at least 30 days according to response and tolerability. [9] This information is prescribing information for physicians on the licensed product. The phase 3 trials used a research formulation; the FDA label presents these results as equivalent doses of the licensed tablet, so the dose names in the articles are not the same as the tablet doses on the label. [9]
Is orforglipron approved?
United States: The FDA approved orforglipron as Foundayo on April 1, 2026. The approved indication is to reduce excess weight and maintain the reduction long term, together with a reduced-calorie diet and increased physical activity, in adults with obesity or in adults with overweight who have at least one weight-related condition. The approval was granted 50 days after submission under the Commissioner’s National Priority Voucher (CNPV) pilot program. [8] In the FDA label revised in July 2026, the indication is weight management only; there is no type 2 diabetes indication, and use together with another GLP-1 receptor agonist is not recommended. [9]
European Union: The EMA’s Committee for Medicinal Products for Human Use (CHMP) gave a positive opinion on October 15, 2026. The proposed indications are weight management in adults with an initial BMI of 30 kg/m² or above, or 27–30 kg/m² with at least one weight-related condition, and, as an addition to diet and physical activity, insufficiently controlled type 2 diabetes. [11] The opinion is an intermediate step; the European Commission makes the EU-wide marketing authorization decision, which normally follows 67 days after the opinion is adopted. [10] [11]
What is the status in Türkiye?
No product named orforglipron or Foundayo appears on the TİTCK (Turkish Medicines and Medical Devices Agency) Licensed Human Medicinal Products List dated October 16, 2026. [12] US approval and the positive opinion in the EU do not mean the product is licensed in Türkiye. Two provisions of the legislation are directly relevant:
- Article 3 of Law No. 1262 on Pharmaceutical and Medicinal Preparations (İspençiyari ve Tıbbi Müstahzarlar Kanunu) requires Ministry permission before domestically produced medicinal preparations are marketed and before those produced abroad are imported. [13]
- Article 24 of Law No. 6197 on Pharmacists and Pharmacies (Eczacılar ve Eczaneler Hakkında Kanun) stipulates that medicines cannot be sold on the internet or through any other electronic medium. [14]
How much weight loss did it produce in obesity?
In both 72-week phase 3 trials, all participants also received diet and physical activity counseling. The percentages in the table come from the “treatment-regimen” analysis, which keeps participants who stopped treatment in their own group. The dose groups are labeled “low, medium, high.”
| Trial | Population studied | Weight change (low / medium / high dose and placebo) |
|---|---|---|
| ATTAIN-1 (NEJM, 2025) | 3,127 adults without diabetes, with BMI ≥30 or 27–30 with at least one weight-related condition; 9 countries | −7.5% / −8.4% / −11.2%; placebo −2.1% [1] |
| ATTAIN-2 (Lancet, 2025) | 1,613 people with type 2 diabetes, BMI ≥27 and HbA1c 7–10%; 10 countries | −5.1% / −7.0% / −9.6%; placebo −2.5% [2] |
In ATTAIN-1, in the highest-dose group, 54.6% of participants lost at least 10% and 36.0% lost at least 15% of body weight; with placebo, these proportions were 12.9% and 5.9%. Waist circumference, systolic blood pressure, triglycerides, and non-HDL cholesterol also improved significantly versus placebo. [1] In ATTAIN-2, all prespecified weight and cardiometabolic measures, including HbA1c, changed significantly in favor of orforglipron. [2]
Two limits matter when reading these results. First, weight loss is smaller in people with type 2 diabetes than in those without diabetes. [2] Second, according to the FDA label, the rate of stopping the study drug for any reason was 22–24% in the orforglipron groups versus 30% with placebo in ATTAIN-1, and 19–22% versus 20% in ATTAIN-2. [9] The manufacturer funded both trials; some of the ATTAIN-2 authors are company employees and shareholders. [1] [2]
What is the effect on blood sugar in type 2 diabetes?
- ACHIEVE-1 (NEJM, 2025): In 559 people with early type 2 diabetes managed with diet and exercise alone and an HbA1c of 7.0–9.5%, HbA1c fell by 1.24 points at the low dose, 1.47 at the medium dose, and 1.48 at the high dose over 40 weeks, versus 0.41 points with placebo. Weight change was −4.5% to −7.6%, versus −1.7% with placebo. No severe hypoglycemia was reported. Permanent discontinuation because of adverse events was 4.4–7.8% with orforglipron and 1.4% with placebo. [3]
- ACHIEVE-3 (Lancet, 2026): In 1,698 people with type 2 diabetes uncontrolled despite metformin, participants received open-label orforglipron at two doses or oral semaglutide at two doses for 52 weeks. HbA1c fell by 1.71 and 1.91 points with orforglipron and by 1.23 and 1.47 points with semaglutide; after orforglipron met the prespecified non-inferiority criterion, it was found superior to both semaglutide doses. By contrast, gastrointestinal adverse events (58–59% versus 37–45%), discontinuation because of adverse events (9–10% versus 4–5%), and the increase in pulse (3.7–4.7 versus 1.0–1.5 beats per minute) were higher with orforglipron. [4]
According to the EMA, the type 2 diabetes assessment rests on four main trials covering 3,765 adults; depending on the dose, orforglipron lowered HbA1c by about 1.2–1.9 points (placebo 0.4–0.8, dapagliflozin 0.8, oral semaglutide 1.2–1.5 points). [10] In the US, this indication is not on the label. [9] HbA1c is a blood marker; these trials were not designed to measure changes in eye, kidney, or nerve complications.
Can weight be maintained after injections?
ATTAIN-MAINTAIN randomized participants who had received injectable tirzepatide (205 people) or semaglutide (171 people) and whose weight had stabilized to orforglipron or placebo. At week 52, those coming from the tirzepatide group maintained, by model estimate, 74.7% of the weight they had lost with orforglipron and 49.2% with placebo (difference 25.5 points; 95% CI 14.5–36.5). Among those coming from the semaglutide group, these proportions were 79.3% and 37.6% (difference 41.7 points; 95% CI 24.4–59.0). [5] The limits noted by the authors are the absence of a comparison arm continuing injectable therapy and the duration of one year; some of the authors are manufacturer employees. [5] Weight regain in the placebo arm shows that a substantial part of the weight can return when GLP-1 therapy is stopped; the course of weight after orforglipron itself is stopped was not the endpoint of this trial.
What is known about cardiovascular and liver safety?
Cardiovascular: ACHIEVE-4 randomized 2,749 people with type 2 diabetes at high cardiovascular risk, with cardiovascular or chronic kidney disease, in 16 countries and regions, in an open-label design, to orforglipron or insulin glargine. Over a median follow-up of 2 years, the primary endpoint, a composite of cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, and hospitalization for unstable angina, occurred in 4.2% of the orforglipron group and 5.0% of the insulin glargine group (HR 0.84; 95% CI 0.59–1.20); the prespecified non-inferiority margin (upper CI bound below 1.8) was met. Serious hypoglycemia was less frequent, and gastrointestinal adverse events more frequent, with orforglipron. [6] This is a safety (non-inferiority) trial; it was not designed to prove cardiovascular benefit. An erratum notice for the article was published on October 17, 2026. [6]
Liver: In a pooled analysis of 11,220 participants (6,920 on orforglipron) across seven phase 3 trials, no orforglipron case met the Hy’s law criteria suggesting severe drug-induced liver injury; liver enzymes decreased on average, and liver-related adverse events were balanced with the comparison groups. Most of the authors are manufacturer employees; people with markedly elevated liver enzymes (ALT/AST at or above 3 times the upper limit of normal in the weight program and above 5 times in the diabetes program) were not enrolled in the trials. [7]
What are the side effects of orforglipron?
The pooled data in the FDA label compare 3,155 patients receiving orforglipron in two obesity trials with placebo. [9]
| Finding (FDA label, pooled two trials) | Orforglipron | Placebo |
|---|---|---|
| Any gastrointestinal side effect | 60–69% (by dose) | 37% |
| Nausea | 26–35% | 10% |
| Vomiting | 13–24% | 4% |
| Constipation | 20–27% | 9% |
| Permanent discontinuation because of side effects | 8% (6–10% by dose) | 3% |
| Tachycardia | 3% | 0.9% |
| Low blood pressure | 2% | 0.5% |
Nausea, vomiting, and diarrhea were more frequent during the dose-escalation period and decreased over time. [9] Acute pancreatitis was seen in 6 patients receiving orforglipron (0.14 per 100 patient-years) and 1 patient receiving placebo (0.04), as adjudicated. Heart rate increased by an average of 4–5 beats per minute (0.5 with placebo). Hair loss was associated with weight loss and was reported in 7% of women and 0.9% of men receiving orforglipron. In people with type 2 diabetes (ATTAIN-2), low blood sugar occurred in 2% with orforglipron and 0.2% with placebo; among those also using a sulfonylurea, the rate rose to 7%. [9]
What warnings does the label carry?
- Thyroid C-cell tumors (boxed warning): Thyroid C-cell tumors have been seen in rodents with other GLP-1 receptor agonists. Orforglipron is not pharmacologically active in rats and mice and did not cause tumors in rodents; it is active at the human GLP-1 receptor, and the relevance of these tumors to humans has not been determined. It is contraindicated with a personal or family history of medullary thyroid carcinoma and in MEN 2 syndrome. [9]
- Other warnings: Acute pancreatitis, severe gastrointestinal reactions (not recommended in severe gastroparesis), acute kidney injury due to volume depletion, hypoglycemia with insulin or sulfonylureas, hypersensitivity, diabetic retinopathy complications in people with type 2 diabetes, acute gallbladder disease, and pulmonary aspiration during general anesthesia or deep sedation. [9]
- Interactions: The highest dose is limited with strong CYP3A4 inhibitors, strong CYP3A4 inducers are avoided, and the simvastatin dose is limited. Because it delays gastric emptying, it can affect the absorption of other oral drugs. [9]
- Pregnancy, breastfeeding, contraception: Animal studies suggest a possible risk to the fetus; the drug is discontinued when pregnancy is recognized. It is not recommended in breastfeeding women. For users of oral contraceptives, the label calls for recommending a switch to a non-oral method, or the addition of a barrier method, for 30 days after initiation and after each dose escalation. Safety and efficacy in children have not been established. [9]

Limits of this guide
This page summarizes published peer-reviewed studies on orforglipron and FDA, EMA, and TİTCK documents. It contains no dosing, storage, or sourcing information and does not recommend use under any circumstances; label information is reported as product information for physicians. For the phase 3 articles, published abstracts were mostly relied on. Because the EU product information (SmPC) will be published after the European Commission decision, the EU information on this page rests on the CHMP opinion and the EMA announcement. If a marketing authorization application or approval occurs in Türkiye, the product information may contain limits different from the trial results on this page.

Frequently asked questions
Is orforglipron approved?
On April 1, 2026, the FDA approved orforglipron as Foundayo for weight management in adults. On October 15, 2026, the EMA's Committee for Medicinal Products for Human Use gave a positive opinion for weight management and type 2 diabetes; the final decision in the EU rests with the European Commission. It is not licensed in Türkiye.
How much weight was lost with orforglipron?
In ATTAIN-1, which enrolled 3,127 adults with obesity, or with overweight plus a weight-related condition, and without diabetes, mean weight change at 72 weeks was −7.5% to −11.2% by dose, versus −2.1% with placebo. In people with type 2 diabetes (ATTAIN-2), the values were −5.1% to −9.6% versus −2.5%.
How does orforglipron differ from semaglutide tablets?
Orforglipron is a small non-peptide molecule and, according to the EMA, can be taken with or without food at any time of day. In ACHIEVE-3, an open-label trial against oral semaglutide in type 2 diabetes, it lowered HbA1c more; gastrointestinal side effects and discontinuation because of side effects were also more common with orforglipron.
What are the most common side effects of orforglipron?
According to the FDA label, the most common side effects are nausea, constipation, diarrhea, vomiting, indigestion, and abdominal pain. In the pooled data from two obesity trials, permanent discontinuation because of side effects was 8% with orforglipron and 3% with placebo; most were gastrointestinal.
Is orforglipron sold in Türkiye?
No. Orforglipron does not appear on the TİTCK (Turkish Medicines and Medical Devices Agency) Licensed Human Medicinal Products List dated October 16, 2026. Law No. 1262 requires Ministry permission for the import and marketing of medicines; Law No. 6197 prohibits the sale of medicines online.
